According to local news sources, an accident that took place in Lucky Friday Mine has allegedly left one out of two victims sustaining highly severe injuries. The authorities got there fast and took the injured victim to the hospital as quickly as possible.

Niho Trucks Are Being Recalled Due to Possible Disruption Risk

Different News outlets have been reporting since early that the commercial trucks manufactured by Hino Motors Sales U.S.A., INC. are being recalled.

According to the announcement, a possible axle assembly malfunction is the reason behind the recall. The manufacturer claims that the model 2012 of the following Hino trucks must be taken fixed immediately: ND8J, NE8J, NJ8J and NV8J.

All of the commercial trucks produced from July through September 2011 may have had a rear axle installation issue during the assembly of the vehicle.

The axle and wheel-end retainer on the front were installed incorrectly by the assembly team of the manufacturer. Because of this issue, the vehicle can lose it’s wheel-hub, which will result in the wheels being disconnected from the vehicle, which can result in several accidents.

Ashland Brand Glass Vase Recalled, Possible Laceration Threat

According to more news and reports regarding product recalls, Michaels Stores have issued a recall for the Glass Vase produced by the Ashland Brand.

The U.S. Consumer Product Safety Commission along with Health Canada announced today that the Ashland™ Glass Vase may turn out to be a laceration hazard. The company stated that after some reports of involuntary glass shattering surfaced, they decided to investigate. Some of the reports stated that a few people got hurt. The U.S. CPSC has ruled the product unsafe for regular use for consumers.

The news report show that approximately 28,000 items must be recalled from both the United States and Canada. The firm stated that the product is made of clear glass and it comes in a rectangular shape. The model number printed on the recalled product is “425827”.

The Michaels Stores firm, which is responsible for distributing the product claims that the vase being recalled can break easily whenever a consumer simply picks them up. Such a frail object shouldn’t be distributed and all of those already sold should be returned immediately.

Pedestrian Accident Claims One Life

According to the Washington State Patrol and other news sources, a 55-year-old woman was allegedly involved in a horrible accident last Saturday. Reports show that she was attempting to cross Highway 99 south of 128th Street SW where there are no pedestrian crossing lines. The causes for her to be crossing the street where she wasn’t supposed to are still unknown. Authorities are still investigating so more can be drawn from this occurring.

The driver who unfortunately hit the woman, a 22-year-old man from Everett, was headed northbound when his vehicle struck the woman. According to an official, full details aren’t entirely available, all that is known is that it seems that she simply threw herself against the car.

Reports also claim that it was raining lightly and the woman was dressing dark clothes. The driver, a 27-year-old man and the 3-year-old passenger are doing fine, the woman however lost her life.

Products from Unsanitary Washington State Facility to be Seized by U.S. Marshals

The U.S. Marshals were requested by the U.S. Food and Drug Administration office to seize food products, according to the news. A Washington-based facility kept a processing and food storage location that was claimed to be unsanitary for the State’s standards. According to official reports, the warehouse was inspected by the FDA. The investigators found a very active infestation of rodents and insects in both warehouse and processing areas.

The first letter issued by the FDA regarding this problem was filed on September 29th. It claimed that the food being kept in the Washington State facility was possibly adulterated, taking into account the Federal Food, Drug and Cosmetic Act. The complaint asked for a speedy action from the Court. A warrant was issued on the same day for the U.S. Marshals to be allowed to take hold of all products present in the facility.

According to the FFDCA, after amended by the FDA Food Safety and Modernization Act, the FDA may order the detaining of all types of food during inspection, specially when food adulteration or misbranding is suspected to have taken place.

Program Will Focus On Making Traffic Safer

Morning news reported that the San Rafael Police Department in California has received a grant for a new program to promote traffic safety. It will go on through the entire year. The project hopes to educate people so injuries and deaths related to traffic won’t happen in the future.

The OTS (Office of Traffic Safety) awarded the San Rafael Police an amount of $129,992 after the city showed they focused all their efforts into a better quality of life and traffic safety measures.

This grant is a proof from the department that their hard work are not in vain. The commitment to maintaining the roads safe through education and enforcement is going stronger than ever.

Reports from the authorities show the number of fatalities related to traffic has lowered in the State of California. Officials say that although DUI deaths have declined a great deal, they remain the number one cause of traffic deaths.

The activities the awarded grant will target are numerous but mostly on motorcycle safety, red light running, drivers with revoked/suspended licenses, DUI offenders and speeding.

According to the Washington State Patrol, a great contributor to accidents on state highways is the ever dangerous drowsy driving. Authorities claim it’s even more of an accident facilitator than drinking and driving because so many motorists simply think they are not going to give in to sleepiness.

The news says the Washington State Patrol has found a new way to crack down this issue by utilizing an easier way to have control over this hazard. Driver fatigue can be fatal but it’s just as hard to be determined whether someone is suffering from it or not.

Accident Involving Two Bicycles and a Vehicle Claims the Lives of Two

According to recent news, two young men were fatally hit while riding their bicycles at nighttime, a third one was also hit but survived and allegedly sent to the hospital with injuries.

Reports from the Department of Public Safety have showed that the young men were wearing helmets and had the bicycle lights on. They were riding their bikes sometime after 7 p.m. when a motorist struck them while trying to pass another vehicle on a rural road.

The 32-year-old driver allegedly fled the scene but later came to his senses and turned himself in. According to the article, the 21-year-old unfortunately passed away after being rushed to the hospital while the 20-year-old rider died at the scene of the accident. The third cyclist, a 19-year-old, was released after being treated for minor injuries.

Washington State Ferry Under Maintenance, Cracks Discovered

According to local news, the ferry Chetzemoka has been found to be unsafe after the Washington State Ferries inspectors noticed dash-like cracks in one propeller. The 64-car ferry might have presented the issue after a stainless steel piece being cast improperly.

After an extensive investigation of the overall state of the one-year-old ferry, the local authorities realized that five different blades were cracking right near the hub. After some tests were run under a metallurgical analyst, reports show that the pieced was actually placed incorrectly as predicted at first glance. Studies also show that the contents of carbon in the stainless steel were extremely high.

The problem with the carbon content being high is that it makes the propellant more fragile to cracks and minor damage. According to the state, the reports show that the propellers are being repaired in a timely manner.
